ASSOCIATION’S AGREEMENT ASSESSMENT SURVEY
The information in this survey is designed to help us find out your general feeling about CWA-AA, USA and most importantly, to help us
increase our membership. Some of the sentences describe your feelings more than others. Please read each sentence and indicate
exactly how you feel by putting an X in the box that best describes your feeling. There is no right or wrong answers. Your answers will be
kept confidential; they would be used to help us constructively revamp the association to increase active membership.  Remember to mark

1. I am an ACTIVE member of CWA-AA, USA.
2. I feel OBLIGATED in some way to CWA.
3. We are going to make PUBLIC our Major Mission
for ’05-’07, things we intend to do and things that
have already been accomplished for CWA. I think this
would help to increase active membership.
4. We are going to PUBLISH brief Financial Information
on our website so that you can be aware of monies
collected and how they are being spent. I think this
would help to increase active membership.
5. We are going to RE-DESIGN some of our programs
and activities for the reunion to help add “FLAVOR” to
our already special events (for all peer groups). I think
this would help to increase active membership.
6. In an effort to achieve our goal of inclusion, we are
going to conduct a massive outreach program to get
everyone involved (from the ‘30’s to ‘00’s). I think this
would help to increase active membership.
7. We are going to create a SUGGESTION BOX to
incorporate YOUR suggestions for better
implementation of our services. I think this would
help to increase active membership.
8. I think this SURVEY is a good start in CWA-AA,
USA’s effort to reach out to me in order to increase
active membership.
9. With my input being considered, I now PLEDGE
my full loyalty and service to CWA-AA, USA & CWA.
